Services count toward annual cap of $3,000 on a per instance amount and may include:

• Bus pass for school-aged children
• School bus pass for 4 months of service maximum
• Driver Education Course – once every 5 years
• License and vehicle registration once per 12 month period

Required Documentation

• ID for anyone over 18 in household (Government issued photo ID /Social Security Card)
• Birth Certificate and Social Security Card for all minors in household
• Verification of all household income for 30 days prior to application
• Child support documents if applicable
• Proof that child is in your household (letter from school showing child name/address, medical bill with child’s name/address, etc.).
• Receipt for reimbursement for Invoice for license and registration fees to pay vendor directly
• Receipt for bus pass
• Receipt for driver education – or invoice to pay vendor directly
