Monetary Cap: $3,000 total, limits on specific services apply
You must apply within 30 days of stated disaster. If you are claiming damages from a natural disaster, you must live in the direct path and have sustained damages from the event.
$3,000 maximum benefit total
- $500 cap on clothing and home supplies (of which only $175 can be for personal hygiene and cleaning supplies). This is paid directly to applicant with screenshots of cart or receipts.
- $1,000 cap on home repair that affects basic structure and/or furniture/appliances. This will be paid to program approved vendors only.
- $1,200 cap on emergency shelter. This will be paid with an invoice to the emergency shelter only.
Required Documentation
- Picture ID for all household members 18 and over (driver’s license, passport, state-issued ID)
- Birth Certificate and Social Security Card for all household members
- Verification of all household income for 30 days prior to application
- Child support documents if applicable
- Proof that child is in your household. Documentation must include the child’s name and the applicant’s address. Pregnant women should provide a doctor’s note. Only validated items – such as SNAP/Medicaid award letters, school correspondence, lease agreement listing children as occupants or medical/immunization records – will be accepted.
- Verification that items are not covered by homeowner’s or renter’s insurance
- Estimates for work to be completed. After application, you will be provided with a list of vendors to contact for estimates to complete work.
- Invoices for items to be purchased or repaired. After application, you will be provided with a list of vendors to contact for purchase of furniture/appliances.
- Receipts of items purchased through grant authorized dealers
- Hotel bill for emergency shelter
- Verification of disaster (Fire Report from fire department, copy of Disaster Declaration from government agency, proof of flood)
