License Reinstatement Assistance is only available if suspension was due to unpaid child support or other Child Support Enforcement Agency (CSEA) guidelines that are now current and CSEA has lifted the suspension.

Suspension cannot be the result of a DUI conviction.

Required Documentation

• Picture ID for all household members 18 and over (driver’s license, passport, state-issued ID)
• Birth Certificate and Social Security Card for all household members
• Verification of all household income for 30 days prior to application
• Child support documents if applicable
• Proof that child is in your household. Documentation must include the child’s name and the applicant’s address. Pregnant women should provide a doctor’s note. Only validated items – such as SNAP/Medicaid award letters, school correspondence, lease agreement listing children as occupants or medical/immunization records – will be accepted.
• Verification of suspension status with CSEA
• Invoice showing amount needed for reinstatement
