This program covers: Repairs to make a vehicle operable
The program does not cover: Cosmetic repairs or body damage that doesn’t limit operability; sales taxes
The vehicle must be the applicant’s sole means of transportation and in the applicant’s name for at least 45 days.
Towing expenses can be considered if the vehicle cannot be driven.
Required Documentation
- Picture ID for all household members 18 and over (driver’s license, passport, state-issued ID)
- Birth Certificate and Social Security Card for all household members
- Verification of all household income for 30 days prior to application
- Child support documents if applicable
- Proof that child is in your household. Documentation must include the child’s name and the applicant’s address. Pregnant women should provide a doctor’s note. Only validated items – such as SNAP/Medicaid award letters, school correspondence, lease agreement listing children as occupants or medical/immunization records – will be accepted.
- Proof of employment or need of transportation for a school aged minor required.
- Two (2) written estimates from a business registered with Ohio Secretary of State. Name of garage, garage phone number and email address must be on the written estimates. Invoice must be within 30 days of application.
- Estimate includes minimum of 30 day warranty
- Valid driver license
- Proof of liability insurance (VIN of vehicle to be repaired must be on the insurance card)
- Proof of ownership (registration, lease agreement)
- Proof repairs cannot be completed by a covered warranty
- If repair is over $1,500 written documentation form vendor that arrangements have been made for remaining balance due
