Monetary Cap: $750

This program covers: Past due utility bills, bulk fuel, installation or repair of medically necessary telephone.

This program does not cover: Emergency shut off prevention, restoration of service, fines or late fees.

Required Documentation

  • ID for anyone over 18 in household (Government issued photo ID /Social Security Card)
  • Birth Certificate and Social Security Card for all minors in household
  • Verification of all household income for 30 days prior to application
  • Child support documents if applicable
  • Proof that child is in your household (letter from school showing child name/address, medical bill with child’s name/address, etc.).
  • Copy of past due utility statements
  • Verification of payment for any amounts over the award cap of $750 and/or fines or fees.
