Must have at least one minor under age 19 enrolled full-time in a K-12 school.

Current school year fees for books, classes, sports, band can be considered.

Clothing and supplies that are necessary for school attendance can be considered.

Required Documentation

• ID for anyone over 18 in household (Government issued photo ID /Social Security Card)
• Birth Certificate and Social Security Card for all minors in household
• Verification of all household income for 30 days prior to application
• Child support documents if applicable
• Proof that child is in your household (letter from school showing child name/address, medical bill with child’s name/address, etc.).
• Invoice from school for fees
• Invoice for clothing such as uniforms
• Print out of shopping cart showing clothing items needed for school from a merchant website (Walmart, Target, JC Penny, TJ Max, etc.)
• Print out Supply List for current grade level for supplies and print of those items in cart  (Walmart, Target, Staples, Office Depot, etc.)
