Rent must be at least 30 days past due to be eligible.

No payments for utilities will be made to landlord unless it is included in lease agreement.

Cannot be living with or employed by landlord.

If full award is not used at time of application, applicant may qualify for additional payments within 60 days of application approval date.

Required Documentation

• Picture ID for all household members 18 and over (driver’s license, passport, state-issued ID)
• Birth Certificate and Social Security Card for all household members
• Verification of all household income for 30 days prior to application
• Child support documents if applicable
• Proof that child is in your household. Documentation must include the child’s name and the applicant’s address. Pregnant women should provide a doctor’s note. Only validated items – such as SNAP/Medicaid award letters, school correspondence, lease agreement listing children as occupants or medical/immunization records – will be accepted.
• Notice of past due rent/eviction notice/other documentation showing month(s) past due and dollar amount owed.
• CURRENT lease that is signed by both parties and dated
• Payment receipt from landlord for verification of any amounts over the service cap.
